Each camper must complete and submit as part of the registration process.

These must be supplied at time of online registration OR via upload through this page, or email to camp admin no less than 2 weeks prior to camp.

2. Camper Pick-up/Sign Out

3. Health History Form

4. Sunscreen Policy

5. Proof of Physical - Must be signed and dated by child’s physician within 18 months of camp

6. Immunization record (from physician) - Must include a record of MMR vaccination

7. Medication Log (if necessary)

8. Authorization to Administer Medication (if necessary)

9. Permission to Administer Epi-pen (if necessary)
